Patient Services
Patient Services is a section provided by the Colorado Adult and Children's Ear, Nose & Throat, P.C. to help make your visit to our office a pleasurable experience. This section enhances the patient-physician relationship by giving you 24-hour access to patient specific instructions and communication tools.

New Patients: We are always happy to accept new patients. Please contact our office to schedule an initial consultation. Once your appointment has been made, please go the the Patient Forms section of this site, print and complete the Patient Demographic Form, Patient Medical History Form and the Consent for Treatment Form. Having these forms filled out before you arrive will help make your first visit a more relaxed experience.
Existing Patients: If you would like to access our Patient Handouts, please click here. We have also made our office forms available to you in the Patient Forms section. You can fill these forms out in advance to save time and bring them with you.
Prescription Refill Policy: If you are in need of a prescription refill, please have your pharmacy fax the request to our office at (303) 347-1140. Please remember in order for your refill request to be processed, your last appointment with us can not be older than 90 days from the refill request date.
Hearing Aids and Ear Plugs: Our office can provide you a wide variety of hearing aids from many different vendors that are competitively priced to beat or meet any similar Hearing Aid you may find outside our office. We also offer many different types of ear plugs that offer different capabilities for different purposes. For example, if you are a band member we can provide you custom musician ear plugs to lower the decibels you experience or we can provide you custom ear plugs for your iPod® to provide a more comfortable fit and will allow you to enjoy your music without having to raise the decibels to an extremely high level due to sound leakage - just ask our licensed Audiologist when you come in about them.
